Music permission for other then on youtube

I was wondering if there is a form or something i need to fill out if i want to use the music on something else other then youtube? Im making a mini-game on minecraft and am wondering if i can use some of the music for a sound pack for the mini-game, and if so what credit should i supply?

The permission form is meant to cover more than just YouTube. If you read the form produced when you fill out your information, you'll see that games are definitely covered in the document.
